Patient Engagement With a Mobile Web-Based Telemonitoring System for Heart Failure Self-Management: A Pilot Study

As this was a pilot study, a convenience sample was used, consisting of patients who had scheduled outpatient visits over the course of a 4-month period between February and May 2013. Patients were deemed eligible if they were ambulatory, English-speaking adults ≥18 years of age with a current diagnosis of heart failure. Patients were required to have regular access to a telephone and be able to navigate a simple website, understand the scope of the study, and provide written informed consent.

These items were rated on a 7-point scale ranging from 0 (very dissatisfied) to 6 (very satisfied). Scores on each subscale were computed by summing the ratings and computing a percentage score ranging from 0 to 100. A composite score was then generated by averaging the 2 percentages, with higher scores indicating greater subjective well-being.

In many instances, primary care visits generate a plan for follow-up actions that extend beyond the encounter, as care often involves tasks that the patient (and sometimes the PCP) will do in the subsequent days and weeks. Ideally, the patient and the PCP jointly decide upon and agree to a plan [4], which might involve initiating or adjusting medications, monitoring symptoms, scheduling tests, implementing lifestyle modifications, or a multitude of other steps.

Successful Organizational Strategies to Sustain Use of A-CHESS: A Mobile Intervention for Individuals With Alcohol Use Disorders

A-CHESS is a mobile phone app designed to help people maintain recovery and prevent relapse from drug or alcohol addiction. In this study, the app was provided to the individual by their treatment agency and included ongoing online and sometimes in-person support from agency counselors. At a minimum, agencies provided connection to a 24-hour crisis line within the A-CHESS app for after-hours coverage. A-CHESS is consistent with self-determination theory and Marlatt’s stages-of-relapse model.
